All InfoMAX services require a 1-year subscription, regardless of monthly payment plan, after which time the subscription will continue to be billed unless client calls Hospice Analytics / Home Health Analytics to cancel. Additional counties may be added to your subscription at any time. Paying monthly, an additional charge for the cost of the counties ordered will occur at time of order (e.g., adding one county would result in an immediate charge of $25), and that charge will then be added to future monthly charges.

**The "fine print": 1) Please see detailed information, demo accounts and videos at www.HospiceAnalytics.com and www.HomeHealthAnalytics.com. InfoMAX counties may include multiple states. 2) Data used include CMS Limited Data Set 100% Standard Analytic Files for hospice, HHA, hospital, and SNF (aka, “claims data”), cost reports, and other public data sources (i.e., no proprietary information is used). All data reported according to CMS reporting guidelines (i.e., aggregate data and cells < 11 are blanked to protect confidentiality). 3) InfoMAX accounts and their related passwords are licensed by Hospice Analytics for the sole use by the "Client of Record" and its authorized employees. Intentional redistribution, sale or access without the written consent of Hospice Analytics is prohibited and may result in account cancelation without refund. 4)The sole remedy for any breach of warranty, expressed or implied, including liability for negligence with respect for services furnished under this order, shall be limited to the re-performance of any defective product and under no circumstances include any incidental or consequential damages.
